How to become a member?

QUALIFICATIONS:

-Must be a natural person

-Must be a Filipino citizen

-Must have attained legal age

-With capacity to contract

-Must reside, work, or have business and other interests in Bukidnon

-Completed the prescribed Pre-Membership Education Seminar

-Must submit ALL of the following documents:

  • Membership Application Form (MAF) with 1×1 ID picture;
  • Duly signed Signature Specimen Card with 1×1 ID picture;
  • PMES Certificate of Completion;
  • Data Privacy Consent Form;
  • Photocopy of any proof of identity such as:
    • Primary IDs are:
      • SSS/GSIS ID
      • Driver’s License ID
      • PRC License ID
      • Passport
      • Digital Comelec ID
      • TIN ID
      • Senior Citizen ID
      • Postal ID
    • Secondary IDs are:
      • Pag-ibig ID
      • Philhealth ID
      • School ID
      • Company ID
  • Duly signed Authority to Conduct CIBI; and
  • Completely filled-up Insurance Form.

-Paid the membership fee, which shall be refunded to the applicant in case of rejection

-Paid the minimum share capital requirement.

-Paid the minimum savings requirement

-Paid the life insurance premium for 1 year.
