QUALIFICATIONS & GENERAL REQUIREMENTS FOR MEMBERSHIP:

1. Completed the prescribed Pre-Membership Education Seminar/Webinar (PMES/PMEW)
2. Must be a natural person;
3. Must be a Filipino citizen;
4. Must be at least eighteen (18) years of age;
5. With the capacity to contract;
6. Within the common bond of membership;
7. Present and/or Permanent Residents of the Philippine Archipelago

NON-MONETARY REQUIREMENTS:

1. Completed the prescribed Pre-Membership Education Seminar/Webinar (PMES/PMEW);
2. Submitted a completely filled-out MEMBERSHIP APPLICATION FORMS (Annexes): 
   -Data Privacy Consent Form;
   -Membership Application form (MAF);
   -Specimen signature card with a recent 1 piece of 1×1 or 2×2 picture;
   -Applicant/Personal Data Sheet (PDS) with a recent 1 piece of 1×1 or 2×2 picture;
   -Permanent residence sketch (incorporated in the PDS);
   -Signed Share Capital Subscription Form;
   -Tax Identification Number (TIN);
   -Photocopy of valid IDs.
3. The prospective member must present and submit at least one (1) primary valid ID or two (2) secondary valid IDs.
   Primary IDs accepted are the following:
   -SSS/GSIS ID
   -Driver’s License ID
   -PRC License ID
   -Passport
   -Digital Comelec ID
   -Senior Citizen ID
   -New Postal ID
   -National ID
   -Voter’s ID or certification
   -Other government issued IDs
   Secondary IDs accepted are the following:
   -Pag-Ibig ID
   -Philhealth ID
   -School ID
   -Company ID
   -Barangay ID
   -4P’s ID
